All first-time patients at Peace of Mind Kankakee should bring the following items with them to their initial appointment, unless otherwise advised by staff:

Current copy of medical records stating the nature of your debilitating condition which qualifies you to participate in the program. Our office will keep this copy in our files in order to properly document your condition. For a list of qualifying conditions, click here.

Current state identification card/driver’s license. If your state identification card or driver’s license does not show your current address, you must bring two proofs of residency.

Veterans of the Armed Forces must bring their DD214/discharge papers to verify their eligibility for a reduced application fee.

Persons who are receiving Social Security Disability Income (SSDI) should bring a Benefit Verification Letter (dated within the last year) from the Social Security Administration. You can get this form by using your My Social Security Account online at https://www.ssa.gov/myaccount. This letter could qualify you for a reduced application fee.
