QUALIFYING CONDITIONS

All new patients at Peace of Mind Kankakee must have a copy of current medical records detailing one of the following debilitating conditions in order to gain approval to the program. 

Please note: If you have a prescription for an opioid pain reliever, or if you could be prescribed one in accordance with a generally accepted standard of care, you automatically qualify for a 90-day card through the Opioid Alternative Program with physician certification. However, if your pain is chronic, we would recommend registering for a 1, 2, or 3 year Medical Cannabis Program card.

  • Agitation of Alzheimer’s disease

  • Anorexia nervosa

  • Autism

  • HIV/AIDS

  • Amyotrophic lateral sclerosis (ALS)

  • Arnold-Chiari malformation

  • Cachexia/Wasting syndrome

  • Cancer

  • Causalgia

  • Chronic inflammatory demyelinating polyneuropathy

  • Crohn’s disease

  • CRPS (complex regional pain syndrome Type II)
