PROOFS OF RESIDENCY

In order to participate in the Medical Cannabis and Opioid Alternative Programs, you must verify that you reside in the State of Illinois.  To verify residency, please bring a current, state-issued identification card or drivers license and ONE of the items listed below (reflecting your current address/name). If your ID card does not reflect your current address/name, please bring TWO of the following items with accurate information to verify residency: 

Please note: The State of Illinois will not allow you to hold a CDL and a medical cannabis registration card. 

  • Utility Bill - Water, cellphone, telephone, electric, gas, and cable are all acceptable. Please be sure that the bill is less than 60 days old.

  • Pay Stub/Electronic Deposit Receipt - must show state income tax withholding and less than 60 days old

  • Voter Registration Card – must still be valid, with address in Illinois

  • Benefit Verification Letter – from the Social Security Administration and dated within the last year. You can get this letter by accessing your My Social Security account online at https://www.ssa.gov/myaccount/. In order to qualify for a reduced application fee, the letter must show disability benefits/income specifically. 

  • Housing Verification - Deed / title, rental agreement/lease, property tax bill, or mortgage statement

  • Bank / Credit card statement – must be less than 60 days old

  • Insurance policy – homeowner's or renter's only

  • Medical Bill, Claim, or Statement of Benefits - from private insurance or government agency, dated less than 90 days old

  • Tuition Invoice – official mail from university/college also acceptable, must be less than 12 months old

  • Notarized Homeless Status Certification - if using this form, this is all you will need to prove residency.
